MEDICAL INFORMATION CARD

Individuals interested in obtaining a medical information card may contact the California Department of Motor Vehicles. The medical information card affixes to the back of a driver's license and allows an individual to record the following information: blood type, allergies, current medication, physician's name and phone number, emergency contacts and organ donation preferences.
